A Clinical Assessment Identifies Your Treatment Needs

An addiction assessment is more than a questionnaire. You meet face-to-face with a licensed clinician who takes time to understand your substance use, mental health, medical history, and the challenges affecting your daily life. This conversation helps build a clear understanding of your recovery needs before treatment planning begins.

During your evaluation, we discuss several areas that help us understand your situation, including:

  • Your alcohol or drug use history, including frequency, duration, and previous treatment.
  • Mental health symptoms such as anxiety, depression, trauma, or bipolar disorder.
  • Current medical conditions, medications, and other health concerns.
  • Your work schedule, family responsibilities, living environment, and support system.

For adults near Orange Township, this evaluation looks beyond substance use alone. We consider how addiction affects your work, relationships, physical health, and daily responsibilities while identifying co-occurring mental health conditions that may also need treatment. This allows us to develop an individualized treatment plan that addresses the factors affecting your recovery rather than applying the same approach to everyone.

Common Questions About Addiction Assessment and Evaluation Near Orange Township, OH

  • Is my addiction assessment and evaluation confidential?

Yes. Your addiction assessment is confidential, and your personal health information is protected by federal and state privacy laws. We will only share your information with your family, employer, the court, or another party if you provide written permission or if disclosure is required by law.

  • Can the assessment be scheduled around work, school, or childcare responsibilities?

Yes. We offer flexible scheduling whenever possible to help adults balance treatment with work, school, childcare, and other responsibilities. During your assessment, we’ll also consider your schedule when recommending the treatment plan and level of care that best fit your clinical needs.

  • What documents should I bring to my addiction assessment appointment?

Bring a photo ID, your insurance information, a list of current medications, and any records from previous addiction or mental health treatment if they’re available. These documents help your clinician better understand your medical history and make more informed treatment recommendations.

  • Will you talk with my family during the assessment if I give permission?

Yes. With your written consent, your clinician may include family members or other trusted support people if their involvement helps provide additional information or supports your treatment planning. Family involvement is always based on your preferences, written consent, and clinical needs.

  • How is an addiction assessment different from a simple screening or online quiz?

An addiction assessment is a comprehensive clinical evaluation completed by a licensed clinician, while a screening or online quiz only indicates whether a substance use problem may exist. Your assessment explores your substance use, mental health, medical history, and daily life to develop an accurate diagnosis, recommend the appropriate level of care, and create a personalized treatment plan.
